James Thurman, DMD

Dr. James C. Thurman has 20 years of experience in implant dentistry. His office was one of the first in Kentucky to use digital x-rays, and he is one of only a few dentists in Kentucky to offer laser dentistry.

Dr. Thurman received his Doctorate of Dental Medicine (D.M.D.) from the University of Louisville in 1976. His post-graduate training included a general dentistry residency at the University of Kentucky/V.A. Hospital as well as training in implant dentistry at Brookdale Medical Center/New York University and at the Medical College of Georgia. He has taken over 1,000 hours of continuing education courses to stay at the cutting edge of the field of dentistry and provide his patients with the very best care.

Steven Welborn, DMD

Dr. Steven M. Welborn earned his Doctorate of Dental Medicine (D.M.D.) from the University of Kentucky in 1970. After graduation he served in the U.S. Army Dental Corps, including a tour of duty at a field hospital in Viet Nam. Dr. Welborn has been practicing in Glasgow, Kentucky for the past 35 years and has had extensive experience in all aspects of general dentistry.

Time away from the office, he enjoys riding his road bike, fishing, reading, and gardening.   When the outdoor weather does not permit, he participates in cycling classes at the local Y.

Amy Norris Cassady, DMD

Dr.  Cassady is a general dentist practicing in her hometown of Glasgow, KY.  She is a 1989 graduate of Glasgow High School and received her Bachelor of Science degree with distinction from the University of Kentucky.  In 1992, she earned her Doctorate of Dental Medicine degree from the University of Kentucky College of Dentistry.  Post-doctoral studies include the completion of a general practice residency program at the University of Alabama/Birmingham.  In 1997, Dr. Cassady entered private practice as an associate of Dr. Norman Gooch in Edmonton, KY.  In 2004, she entered into the dental practice of Drs. Thurman, Welborn, and Cassady.

Dr. Cassady has devoted her professional career to all aspects of general dentistry providing comprehensive care to her patients.  She has special interests in root canal therapy, cosmetic dentistry, oral surgery, and pediatrics.  To keep abreast of the latest developments in dentistry, she has always exceeded the continuing education requirements of Kentucky.

Married since 1996, Dr. Cassady and her husband, Craig (who is also a Glasgow native), have 3 children:  Parker, Connor, and Mia.  When not working, she enjoys spending time with family, exercising at the local Y, attending her childrens’ various sporting events and activities, and reading.

Gina Miller, RDH, BSDH

Gina graduated from Western Kentucky University in 1997 with an Associate of Science, Dental Hygiene degree.  She completed the Bachelor of Science, Dental Hygiene degree the following year, 1998.  Gina joined our practice in April of 1998.  She has worked in dentistry for over 20 years, as a dental assistant before obtaining her RDH license.  Gina has been an active member of her professional organization, holding various offices for the South Central Kentucky Dental Hygienists’ Association for a number of years and also serving on the Executive Board for the Kentucky Dental Hygienists’ Association from 1999 through 2007.  She is also a past President of KyDHA.  Gina  has her license to administer block and infiltration anesthesia and nitrous oxide analgesia.

Gina is marred to Dr. Dewayne Miller, physical therapist and clinical director of KORT, Glasgow Physical Therapy.  They live in Glasgow with two of their three daughters, Hannah and Emma.  Their oldest daughter, Brittany, is attending WKU.  When not working, Gina enjoys spending time with her family.  She is an avid reader, crafter, and a self-proclaimed amateur photographer.


Angela Jaworsky, RDH

Angela graduated from Western Kentucky University in May 2011 and has been a registered dental hygienist on our team since September of 2011.  She is a new resident to the city of Glasgow, previously residing in Bowling Green.  During hygiene school, Angela traveled to Belize providing dental care to those less fortunate, and also participated on the WKU Mobile Health Unit, visiting local elementary schools to provide oral health care to children.   Attending continuing education courses is high on Angela’s priority list so she can stay up- to- date with the newest information and techniques to provide our patients with the best dental care possible.  Angela has her license to administer block and infiltration anesthesia and nitrous oxide analgesia .  . .   . .

In her free time, she enjoys reading, playing the flute, painting, and spending time with her pet cat. 

Sandra Stone, RDA, CDA, EDDA

Sandra is a certified and registered dental assistant and expanded duties dental assistant.  She graduated from Bowling Green Technical School in 1986 with certification in dental assisting.  She then completed expanded functions from the University of Louisville.  She has been a member of DANB since 1986.

Sandra joined the practice in September of 1991.  She loves helping others and gains a great deal of satisfaction in providing quality care to our patients.

In addition to working at the dental office, Sandra and her husband, Michael, own a local business, Mobile 1 Express.  They have two children, Corey and Collin.  Sandra loves spending time with family and her hobbies include art, crafts, and thrifting.


Heather Hammer, RDA, EDDA

Heather is a registered dental assistant and expanded duties dental assistant obtaining certification from the University of Kentucky.  She has 18 years of experience assisting and joined our practice in 1999. 

Heather is a people-person and loves interacting directly with our patients.  She has been married for 19 years to her husband, Bart, and they have 2 children, Tyler and Hadleigh. In her spare time, Heather can be found at a ballpark or soccer field cheering on her children.  She loves arts and crafts and even has her own line of handmade scarves and wooden signs. 


Annie Thompson, RDA, EDDA, Purchasing Coordinator

Annie is a registered dental assistant and expanded duties dental assistant who joined the practice in 1998. She began her expanded functions in 2008 and has maintained the role of purchasing coordinator since 2000. 

Annie has been married for 9 years to Scott, and they have one son, Kellen.  They reside in Horse Cave, KY. The family are huge Kentucky Wildcat fans who enjoy nothing better than to follow the Wildcats!



Chassidy Elmore, RDA, EDDA

Chassidy is a registered dental assistant and expanded duties dental assistant.  She studied expanded functions at the University of Kentucky and has been assisting since 2006.  She joined the practice in 2010 relocating back to her hometown of Glasgow from Columbus, GA.  

When away from the office, Chassidy enjoys exercising, shopping, dancing, and listening to music.
