Patient Forms, Map and Directions to Glasgow Office
Your initial oral examination will involve a clinical examination by the dentist, charting of the existing dental work, diagnoses of existing conditions, and discussion of treatment recommendations and treatment options. A custom treatment plan will be devised and given to you. In order to properly perform a comprehensive examination, we will take x-rays, which may include the panoramic x-ray which gives us an overall view and the bite-wing x-ray series for proper diagnosis of proximal decay of posterior teeth. For most adults, a full mouth series will be deemed necessary for proper diagnosis. Complimentary intraoral photos may be taken to give you a visual of your existing condition(s). Our office is on the leading edge of technology utilizing digital radiography and paperless charts.
Please be prepared to give us a list of medications you are taking and to answer pertinent information regarding your medical history.
Our practice is working together to realize a shared vision of uncompromising excellence in dentistry.
To fulfill this mission, we are committed to:
- Listening to those we are privileged to serve.
- Earn the trust and respect of patients, profession and community.
- Exceed your expectations.
